Dementia Community Support Scheme

Objective

DCSS involves Hospital Authority (HA), Social Welfare Department (SWD) and non-governmental organisations (NGOs) to enhance support services for elderly persons with mild or moderate dementia.

 

On the basis of dementia care framework, the implementation of Dementia Community Support Scheme (DCSS) aims at establishing a sustainable and effective medical-social collaboration model with multi-disciplinary inputs to enhance the community support for the well-being of elderly persons with mild or moderate dementia in Hong Kong

 

Under DCSS, elderly persons with mild or moderate dementia and their carers are arranged to attend structured intervention programmes at the District Elderly Community Centre (DECC) with an aim to delay their functional and cognitive decline.

 

Target Users

Target users of the DCSS are aged 60 or above and;

(a) patients diagnosed with mild or moderate dementia and referred by HA or

(b) members of respective  DECC who are suspected of having features of early dementia.

 

Priority of referrals would be given to HA’s patients with mild dementia.

 

Content

We will provide 5-9 months of Integrated Training Programmes to the service user and the carer according to the Integrated Care Plan formulated by the medical-social collaboration platform.

 

The core training modules include cognitive module, functioning module, psychosocial module and carer burden module.

 

Additional training modules including BPSD module and physical co-morbidities module, or booster training can be provided according to individual needs of patients and carers (if available).

 

Service fee

Participants who are recipients of Comprehensive Social Security Assistance (CSSA) or Old Age Living Allowance, or holders of medical fee waiver (not applicable to holders of one-off medical waiver) can receive free services under the Scheme.

 

Participants who are neither recipients of CSSA or Old Age Living Allowance, nor holders of medical fee waiver can pay a monthly fee of $150 to receive support services in that month under the Scheme.

 

Participants may consider using Elderly Health Care Voucher Scheme (EHCVS) to pay the monthly fees.

Application method

DECC members who are suspected of having features of early dementia,

  1. Elderly persons diagnosed with mild or moderate dementia using HA Geriatric or Psycho-geriatric services can be referred by HA; or
  2. DECC members who are suspected of having features of early dementia, please contact the respective DECC.

 

Withdrawal and Termination

  1. Case will be closed if the patient and/ or the carer (if available) indicate their withdrawal (i.e. self-withdrawal) from participating in the DCSS, either verbally or in writing.
  2. Cases that are unreachable, or unable to attend the scheduled training programme for a considerable period of time (e.g. two months) due to special reasons (e.g. prolonged hospitalisation) will be considered to close after discussion with HA.
