Objective

We are devoted to provide community support service to elders and hope they are being respected and care, thus, stay in familiar community and enjoy aging.

 

Goal

  • To encourage the elders to make optimum use of their leisure time, to have lifelong learning, develop their potentials and enjoy active aging. 
  • To promote healthy life style and holistic developments of the elders, in physical, psychological and social aspects. 
  • To provide community support services so as to enhance the quality of the elders' life

Service Target

Eligibility of membership: 

  • Member: Any person aged 55 or above and holder of a valid HKID card 
  • Carer member: Any caregiver of elderly aged 18 or above  

Casework services:

  • Counselling and Outreach Support Service: Any person aged 60 or above and living in designated service area

 

Our Services

Dementia Care Services 

  1. In-Home Cognitive Training 
  2. Mini Day Care Service 
  3. Other Services such as service inquiries, initial assessment, referral and mutual support groups 

 

Support Services for Hidden Elders 

  1. No Worry Link : The service proactively ensure the safety of deprived elderly singleton or patients with chronic or terminal illnesses by scheduled daily telephone contacts. 
  2. Volunteers caring visits/calls 
  3. Social gatherings  

 

Centre-based Services 

  1. CareAge Nurse Station 
    • Serving aged 60 or above and their carers, providing health check, medical and illness consultation, health education etc.   
  2. Meal Services 
    • Providing nutritious meals to aged 60 or above elders with dietary needs.

Carer Support Services 

  • Helping carers to cope with their difficulties and sudden caring needs encountered.

Mon - Fri: 8:00 a.m.-6:00 p.m. 

Sat & Sun: 8:30 a.m.-5:00 p.m. 

Public holiday: Closed 

 

Application Method

  • During office hours, bring along the identity card, come to our centre in person, pay the member annual fee, the membership will be valid for one year (from April 1 every year to March 31 of next year) 
  • People who want to withdraw can send us a letter, come to our office in person or by phone, the member fee will not be returned 
  • For those haven't continue the membership within the scheduled period, will be regarded as withdrawn automatically. 

 

Fee & Time of fees collection

  • Annual Membership Fee $21 (from April 1 every year to March 31 of next year) 
  • Time of fees collection
    • Monday to Friday: 8:30 a.m. - 5:30 p.m.
    • Saturday & Sunday: 9:00 a.m.- 5:00 p.m.
