Stroke Recovery Program

Objective

  1. Promote early rehabilitation and exercise programme
  2. Improve self-management skills of daily activity living 
  3. Provide necessary support for stroke survivor’s carer

 

Target Group

  1. Stroke survivor (Priority given to 1 year post stroke)
  2. Referral from doctor, nurse, medical social worker, physiotherapist, occupational therapist or community social worker. 
  3. Financially deprived. Assessment from social worker is necessary, requirement including the following:
    • Old Age Living Allowance OR
    • Working Family Allowance (WFA) Scheme OR
    • Monthly household income not higher than 70 per cent Medial Monthly Domestic Household Income of economically active households (MMDHIEA) OR
    • Comprehensive Social Security Assistance (CSSA) Scheme

 

Services

Professional services including the following:

Physical:

  1. Tailor-made rehabilitation training
  2. Nutrition guidance and body health monitoring

Mental:

  1. Counselling and emotional support
  2. Stroke survivor’s carer support group

Community:

  1. Home environment assessment and modification
  2. In-home rehabilitation training  

 

The team consists of physiotherapist, occupational therapist, and social worker. Referral for other healthcare professionals will be made if necessary.

Transportation can be arranged in Hong Kong island accordingly.

 

Service Hour

Monday to Friday 9am – 6pm

 

Application Method

Referral from hospital doctor, nurse, medical social worker, physiotherapist, occupational therapist or community social worker is necessary.
