Central & Western Integrated Home Care Services Team

Objectives

To provide comprehensive care and support services to the elderly persons, people with disabilities, and individuals and families with social needs living in the community, in order to promote “ageing in place” and “continuum of care” service concepts.

 

Goals

Our teams will provide appropriate services according to the individual needs of service users, striving to enable them to live comfortably in the community and providing support services to carers to alleviate their care pressure.

 

Target Groups

  • Carers
  • People with disabilities or illnesses
  • Individuals and Families with social needs
  • Aged 60 or above living in the community and not receiving institutional care

 

Types of Cases

Mild Impairment Cases

Assessed to be at the state of mild impairment or above under a designated assessment tool.

 

Ordinary Cases

Priority will be given to individuals and families with weak personal and family support and low-income.

 

Frail Cases

Assessed and recommended for Community Care Services or Residential Care Services under the Standardised Care Need Assessment Mechanism for Elderly Services.

 

Fees

Mild Impairment Cases

Service users are required to make co-payment with amounts determined on the basis of their household income, and may be revised annually.

 

Ordinary Cases and Frail Cases

Fees are charged on a sliding scale with reference to the service users’ financial conditions and may be revised annually.

Services provided to mild impairment cases include

  • Personal care
  • Household cleaning service
  • Escort Service
  • Meal service
  • Basic nursing care
  • General physical exercises
  • Elderly sitting
  • Purchase and delivery of daily necessities
  • Environment risk assessment and home modification
  • Other services, e.g. carer support service, etc.

 

Services provided to ordinary cases include

  • Personal care
  • Escort Service
  • Meal service
  • Basic nursing care
  • Child care
  • Household cleaning service
  • Elderly sitting
  • Purchase and delivery of daily necessities
  • Environment risk assessment and home modification
  • Other services, e.g. carer support service, etc.

 

Services provided to frail cases include

  • Personal care
  • Rehabilitation exercises
  • Escort Service
  • Meal service
  • Counselling service
  • Speech therapy
  • Elderly Sitting
  • Household cleaning service
  • Arranging respite service
  • Care management and assessment
  • Basic and special nursing care
  • 24-hour emergency support
  • Carer support and training
  • Environmental risk assessment and home modification

The scope and frequency of services will be designed by the IHCS Teams according to the needs of the service users.

 

Application

Mild Impairment Cases

  • Elderly persons who are on the waiting list for Integrated Home Care Services (Ordinary Cases) and assessed to be at the state of mild impairment or above under a designated assessment tool ; or
  • Assessed and recommended for “Home Support Services” under the Standardised Care Need Assessment Mechanism for Elderly Services, and referred to the respective IHCS team by social worker.

 

Ordinary Cases

Applications can be made by phone or in person during office hours, or through referrals from social workers.

 

Frail Cases

Applications should be assessed and recommended for Community Care Services under the Standardised Care Need Assessment Mechanism for Elderly Services, or referred by residential care services.

 

Withdrawal / Termination

Mild Impairment and Ordinary Cases

  • Case managers will review service needs regularly to adjust or terminate the service according to the conditions of the service users.
  • Service user can request for termination of service at any time.

 

Frail Cases

  • Service user can request for termination of service at any time.
  • Service user will be treated as withdrawal of service automatically by leaving service for over 30 days or moving to a residential care home in accordance with the stipulation of Social Welfare Department.

Monday to Friday : 8:30AM to 6:30PM
Saturday : 9:00AM to 6:00PM
Lunch Time : 1:00PM to 2:00PM
Closed on Sunday and Public Holidays
