Central and Western Enhanced Home and Community Care Services

Service Objectives

To actualize the concepts of “ageing in place” and “continuum of care” through the provision of full spectrum of care and support services, to assist frail elderly persons to continue living in the community for as long as possible, and maintaining their optimal level of functioning.

 

Service Goals

EHCCS Teams will provide various kinds of services according to our service users’ individual needs to facilitate their adaptability to continue to live in the community. Meanwhile, we provide diversified support and assistance to the carers to overcome all sorts of caring difficulties and relieve caring pressure.

 

Target Groups

  • Elderly Persons
  • Aged 60 or above, living in the community and not receiving institutional service; and
  • Assessed and recommended for Community Care Services or Residential Care Services under the Standardised Care Need Assessment Mechanism for Elderly Services.
  • Carers

 

Fees

Fees may be revised annually and are charged on a sliding scale with reference to the service users’ financial conditions.

The scope and frequency of services will be designed by the EHCCS Teams according to the needs of the service users.

 

Application

Elders or their carers can approach service units below for enquiry, application or referral:

  • Integrated Family Service Centres / Integrated Services Centres
  • Medical Social Services Units
  • District Elderly Community Centres / Neighbourhood Elderly Centres / Social Centres for the Elderly
  • Hotline of Social Welfare Department: 2343 2255 (For enquiry only)

 

Withdrawal/Termination

  • Service user can request our service teams for termination of service at any time.
  • Service user will be treated as withdrawal of service automatically by leaving service for over 30 days.
  • Service user is going to move to a residential care home.

 

Service Hours

Monday to Saturday: 9:00AM to 6:00PM

Lunch Time: 1:00PM to 2:00PM

Closed on Sunday and Public Holidays
